As a head coach you are responsible and should have reviewed with your manager all your players ID and Date of Birth from legal documents provided as per HFA and OFL rules.
The head coach and parent/guardian must sign the waiver form acknowledging any players that are under age and that both you and the parent are aware as well as approved for play on your team.
The Form can be downloaded below.
It must be submitted to HFA prior to season game play. Keep a copy for your sideline binder as well.
During this process if you have identified errors on a DOB from the registration list we provided you please make us aware. We do not keep official DOB documents on file and only go by parents honour system via online registration.
You are responsible on behalf of HFA to confirm Date of Birth via a legal document of some kind.
NO players are allowed to play down a age group. Be sure to review your players official legal documentation (passport, Birth Certificate etc) for proper age and that matches the DOB the provided on registration system and fits your league age division requirements.
